Long-Term Care Counseling

In addition to providing you with quality information on available services and benefits for you or your loved one, the Aging & Disability Resource Center also offers you the option of one-on-one long-term care counseling.  Whether you are beginning to notice changes in ability to carry out daily tasks, are noticing changes in memory, or are facing the onset of a disability or disease, we will provide you with all of your options and help to guide you through the decision making process. 

 When appropriate we can screen for eligibility for the long-term-care programs.  These programs, which include; the Community Options Program (COP), the Community Integration Program (CIP), and the Alzheimer’s Family Caregiver Support Program (AFCSP) help to support the elderly or disabled individual to reside and remain independent in the community.  We can offer a full, comprehensive assessment of the individual’s needs including screening the individual’s functional abilities as well as their financial eligibility.  The Resource Center is here to help guide you through the difficulties and stresses that can occur when facing difficult decisions regarding your care or the care of a loved one.
