We, the staff of the Developmental Disabilities Unit,
thank you for your interest in our services. We coordinate community resources
to provide for the needs of individuals with developmental disabilities (the consumer),
from birth through adulthood, and their families. A developmental disability is
defined as: mental retardation, epilepsy, cerebral palsy, autism and
traumatic brain injury prior to the age of 22.
We coordinate services with other Green County Human
Services Units to access County, State and Federal programs and funding.
Services offered include case management, needs assessment, treatment plans,
assistance obtaining Social Security, payee services, protective placements and
guardianships.
Our unit provides services through the Community
Integration Program (CIP 1a and CIP 1b) and the Community Options Program (COP).
County, State and Federal funds help pay for services for eligible individuals
with developmental disabilities. Fees or cost shares may apply in certain
circumstances.
A psychological or psychiatric evaluation may be
required to determine whether a person is eligible for Developmental
Disabilities services and whether the person is eligible for Social Security
benefits.
Depending upon the availability
of funds, the following services may be provided through the Green County
Developmental Disabilities Unit:
Through our Aging & Disability Resource Center we
can provide information about agencies and services available for people with
disabilities, and may be able to make referrals for services, when appropriate.

Follow-Along
Services
Case
management and service coordination assures the consumer receives a
full range of appropriate services in a planned, coordinated,
efficient and effective manner and includes planning for long-term
needs, advocacy, counseling, assessment and referrals to other
services.

Diagnostic
Services
Referral to a physician to identify the presence of a
developmental disability.

Evaluation
Services
We can conduct an evaluation of the strengths and needs of an
individual in order to develop a comprehensive treatment and service
plan.

Counseling
Services
We can provide referral for individual and group
counseling specifically targeted towards individuals with
developmental disabilities.

Education
Services
Prevention
is an important part of the Developmental Disabilities Unit. The Green
County Birth to 3 Program provides services for children in the family
home assessed with a developmental delay or at risk of a
developmental delay. When the child becomes 3 years old, she/he may transition
into a public school Early Childhood class, if eligible. Case
management may be available for children with a developmental
disability age three to graduation.
Case
managers participate in the IEP process with public schools to assist
students with developmental disabilities transition from school
into adult services.

Recreational
Services
Assistance
with developing skills and interests leading to enjoyable and
constructive use of leisure time and improved well-being.

Training
Services
Training
in activities of daily and community living, such as self-help,
communication and motor skills can be provided through contract
services.

Treatment
Services
We
can make medical or medically related referrals related to conditions
of developmental disabilities.

Special
Living Arrangements
We also plan, arrange and monitor placement in
Adult Family Homes, Community-Based Residential Facilities and
independent living situations.

Transportation
Services
Transportation
for persons with a developmental disability may be provided for
sheltered employment and work activity, medical care and special
programs.

Protective
Services
We
can provide assistance to people with developmental disabilities who
are unable to manage their own resources or to protect themselves from
neglect, abuse, exploitation or degrading treatment and help them
exercise their rights as citizens.

Sheltered
Employment & Work Activity
Opportunities
to learn work skills and supported employment are provided through a
contract agency in the area. We can also coordinate services through
the State Department of Vocational Rehabilitation (DVR).

Adult
Day Care
Referrals
can be made to adult day care for eligible individuals.

Personal
Care
We coordinate services with contract agencies for
respite, supervision and chore services and assistance with personal
hygiene and activities of daily living.

Domiciliary
Care Services
We
monitor the care and treatment of Green County residents living in the
state centers for people with developmental disabilities, nursing
homes and intermediate care facilities for the mentally retarded (ICF-MR).

Birth to Three**
We provide services and therapy for infants
and toddlers with a developmental disability or developmental delay.
We provide transition services for children entering early childhood
in the public school system at age three.

Family Support Program
Provides funding for goods and services, which
cannot be obtained through any other funding source. This program is
for children living in their family home and in school. Children are
eligible until age 18 (age 22, if still in public school).
