Community
Based Long-Term Care For Elderly & Physically Disabled The Unit provides a
variety of community and home based services to physically disabled adults and
elderly who are experiencing long-term chronic disability to enable them to
live as independently as feasible in the community.
The unit offers a comprehensive assessment and works with the
individual to develop a case plan which identifies the services
required to meet the individual=s
needs and preferences. Identified services are provided and arranged for with State
and Federal long-term care funding (COP, CIP, AFCSP), other community
resources and utilization of the natural support system.
Services provided may include housekeeping, personal care, home
delivered meals, transportation, adaptive aids, home modifications, respite
care, guardianship/protective placement services, residential care options and
day programming. In order to
qualify for those services, the individual must be functionally and
financially eligible. Guardianship And
Protective Service/Placement When an individual is
believed to no longer be capable of making decisions for themselves, a
guardianship or guardianship and protective service /placement may be
appointed/ordered by the Elder Abuse and
Neglect The Unit is the designated
county point for receiving concerns regarding potential elder abuse and
neglect situations. A social
worker investigates the concerns and offers services to relieve the situation. Click here to download the "What You Should Know About Elder Abuse" brochure. You will need Adobe

Payee/Budget Counseling Upon the determination of
the Social Security Administration, individuals who are not capable of
appropriately managing their Social Security or SSI benefits and who have no
other family/interested party to help may secure a representative payee to
assist with money management. Short-term
voluntary budget counseling is also available from the Unit. Aging
and Disability Resource Center The Resource Center is
available to provide accurate and useful information about services, benefits
and community resources for older adults, people with physical disabilities,
people with developmental disabilities and their families.
Through assessment of the caller=s
needs, Resource Center staff will assist in connecting the individual to
appropriate agencies and service providers in and outside of Green County
Human Services. Programs
Operated Through the Adult & Aging Unit
The opportunity to join
with other individuals age 60 and over to share a nutritious noon meal is
available at the following locations: Monticello (Gempeler=s Supermarket), New Glarus (New Glarus Home AGreen
Room@),
Belleville (St. Mary=s
Church and Sugarwood Apartments), Albany (Riverview Apartments), Brodhead
(Brodhead Senior Center), and Monroe (Monroe Senior Center).
A
hot, nutritious meal is delivered around noontime to individuals who are age 60
and over and who are homebound as the result of illness, frailty, and/or disability.
Meals are delivered from the Congregate locations identified above.
Home Delivered Meals serve as a source of nutrition, provide a Asafety
check@
Mondays through Fridays, and offer a social contact between volunteers and
participants.
Driver escort transportation
services are provided by screened volunteers who provide transportation using
their own vehicles for medical, nutrition, business, and social transportation
in that order of priority. Van
transportation (including two vehicles which are handicapped accessible) are
utilized to transport individuals to Hand In Hand Adult Day Center, to provide
shuttle transportation from outlying areas to major shopping areas and for van
riders to pursue whatever proposed activity they arrange or desire. The Aging Unit works with
the Economic Support Unit to arrange for Medical Assistance funded
transportation.
A
highly trained staff person who has as access to consultation with Elder
Law Attorneys provides benefits counseling to individuals who are age 60 and
over and/or their families. Assistance
is provided at the office, in homebound individuals=
homes, and at outreach sites throughout the county.
An
every other month newsletter which is distributed to 1,850 elderly households
provides timely information regarding benefit updates, consumer affairs,
upcoming activities, health promotion information, and schedules for meals and
transportation.
A twice-a-week social model
respite program for individuals in need of ongoing supervision and/or who are
isolated and/or who need assistance with activities of daily living.
Transportation, a noon meal, snacks, activities, field trips, support to
caregivers, health promotion information, and social time with peers are
services provided. The program
gives caregivers Atime
off.@
Activities
or events which educate and promote wellness are planned and implemented.
A minimal amount of funding
is available to assist individuals age 60 and over who are on waiting lists or
who have needs that are not ongoing, such as for adaptive equipment, lifeline
deposit, one-time housekeeping assistance.
A minimal amount of funding is also available to help caregivers.
The Aging Unit has one Wise
Options slot which provides employment and training for an individual age 55 or
over who meets specific eligibility criteria.
